The major scientific achievements of the Institute for 10 years are briefly described in the article. These are the results of the inventive activity and the latest technology in the sphere of the healthcare. Despite economical and social difficulties the Institute of tuberculosis and pulmonology remains a leading center of scientific, research, clinical, organizational and methodical work in Ukraine.

The state of modern phthisiology and a brief review of ongoing studies at the Institute of tuberculosis and pulmonology during 1991-2000 are reflected in the article. The causes of a growth of TB morbidity and mortality are analyzed.

The sequential therapy of 30 patients with nosocomial pneumonia with ofloxacin (intravenous injection of 0,4 g of ofloxacin for 2-3 days followed by oral ofloxacin 0,4 g twice daily) helped to achieve high clinical effectiveness of treatment, even when ofloxacin had been administered as monotherapy.

The combinations of 5-6 antituberculous drugs (INH, RMP, SM, PZA, ETB) were applied during all or nearly all 5-months course of treatment in 97 newly diagnosed patients with destructive pulmonary tuberculosis. The drugs were administered daily or mainly daily, i.e. 5 drugs per day. Sputum abacillation was achieved in 100% of cases on average in 1,54Б0,1 months, caverns healed in 93,8% of patients on average in 3,21Б0,19 months. An achieved effect was enough sustained. Adverse events occurred in 25,8% of patients (serious - in 6,2%).
In comparison with standard intensive chemotherapy, consisting of 4 drugs per day during first 2-3 months, a developed method reduced terms of abacillation on 0,35 month, terms of caverns healing and hospital treatment on 1 month. A total length of chemotherapy decreased on 1,5 month. A rate of abacillation and caverns healing in both regimens, as well as a tolerance didn't differ significantly. A proposed polychemotherapy regimen permitted to reduce a total duration of chemotherapy up to 5 months in this group of patients due to faster abacillation and caverns healing in first 2-5 months of treatment.

The effectiveness of complex ethiopatogenetic therapy in patients with newly diagnosed destructive pulmonary tuberculosis, exposed to small doses of radiation after Chernobyl atomic power station disaster, was evaluated.
In patients, exposed to small doses of radiation, an overall clinical effectiveness of treatment was 13,98% lower than in not-exposed patients (82,14% vs. 96,12%). The pathogenesis therapy helped to shorten detoxication period, normalized total blood count, protein blood composition, improved immunity indices, decreased inflammation in the lungs and bronchi, improved a tolerability of antituberculosis drugs. The pathogenesis therapy did not influence the frequency of abacillation.

The dynamic follow up on liquidators, who took part in Chernobyl post accidental work, testifies an increase of prevalence of bronchopulmonary diseases in comparison to a control group. A trend towards chronic bronchitis predominance in each stage of a disease has been noted. The chronic bronchitis remains a challenge, requiring an elaboration of adequate diagnostic methods, applicable in an outpatient practice.

The results of comparative evaluation of efficiency of chemotherapy regimens plus extracorporal procedures (ultraviolet blood radiation in combination with plasmapheresis or combined laser therapy) and chemotherapy alone in patients with infiltrative pulmonary tuberculosis in phase of destruction are presented. The influence of therapies on pulmonary function indices and pulmonary circulation has been studied. The application of extracorporal procedures in complex therapy of patients with infiltrative pulmonary tuberculosis in phase of destruction increased the effectiveness of treatment.

There were analized 267 cases of active pulmonary tuberculosis in children. There was established that exacerbation of pulmonary tuberculosis occured in 6% of children during hospital treatment. There were described the peculiarities of course, causative and risk factors of pulmonary tuberculosis. There was underlined the importance of BCG vaccination for prevention of tuberculosis and and its complications, particularly in tuberculosis contacts.

The schoolchildren with doubtful results of 2 TU PPD-L Mantoux skin test were followed up for 3 years. A doubtful reaction was registered in 36,7% of cases; in 47,6% of cases only hyperemia was observed. It was concluded that children with doubtful tuberculin reaction aged 5-7 years had to be considered a high tuberculosis infection risk group, because in 33,3% of them a tuberculin test conversion would occur within 2 years plus more 12,5% - within next following 3 years. About 75% of these children would be infected during 4 years after doubtful tuberculin reaction had been revealed. Thus, they need strict follow-up checks with three Mantoux tests within the first year. If positive skin reaction in Mantoux test do not occur, a BCG revaccination must be considered.

Clinical efficacy of ahistam in treatment of allergic rhinitis in children

The clinical efficacy of second generation H1-histamine receptor blocker ahistam was studied in 25 children with allergic rhinitis. This medication was effective in treatment of patients with seasonal and all-year-round rhinitis and also in cases of rhinitis with concomitant asthma. Ahistam had good tolerability, no toxic effects were registered in children.

The influence of female steroid hormones on the bronchial mucous membrane fibrinolytic potential in patients with chronic obstructive bronchitis associated with hypoestrogenia

Using in vitro experimental models there was established an existence of hormone dependent controlling system of bronchial mucous membrane fibrinolytic activity in females with chronic obstructive bronchitis (COB) and concomitant hypoestrogenia. The results of the study created a basis for an evaluation of effectiveness of substitutive estrogen therapy in COB patients in menopause and postmenopausal period.

The system was developed for formalized and unified description of patient clinical state, for elaboration of computer diagnosing methods and disease prognosis. Besides, this system included the tools for creation of common fragments in standardized medical information database.

In newly diagnosed tuberculosis patients, admitted to one of the city antituberculosis dispensary department we conducted a questionnaire-based survey with an aim to reveal a hygienic knowledge of patients about tuberculosis. The level of knowledge was very low, significantly affecting patients' attitude to the therapy procedures. To extend patients' information about tuberculosis there was created the Robert Koch school, where the doctors using a special program, conducted the essential lessons on tuberculosis.
The individual work with the patients at R. Koch School had a positive influence on the effectiveness of treatment and patients' compliance.
