COMMUNITY-ACQUIRED PNEUMONIA IN ADULTS (ETIOLOGY, PATHOGENESIS, CLASSIFICATION, DIAGNOSTICS AND ANTIBACTERIAL THERAPY) (Project of recommendations)

The methodical recommendations (with changes and adding to recommendation which were published in 2001) are devoted to the relevant problem of respiratory medicine - community-acquired pneumonia in adults. The up-to-date definition of pneumonia, epidemiological data regarding its prevalence, mortality and lethality have been presented. The etiology, pathogenesis and diagnostics of the disease have been characterized. There have been described the modern approaches in treatment of community-acquired pneumonia. A special emphasis has been put on the rational administration of the antibiotics. The authors have summarized the data regarding novel antibacterial medications and treatment regimens. The effectiveness of therapy evaluation criteria and algorithms of treatment community-acquired pneumonia has been stated.

EFFICACY OF AMPICILLIN / SULBACTAM IN TREATMENT OF CHILDREN WITH RESPIRATORY INFECTIONS

In traced study was established, that therapy ampicillin/sulbactam (in dose 150 mg/kg in 24 hours divide on 4 intramuscular injections on 10 days) was highlyeffective in threatment of community-aquired pneumonia and recurrence tonsillitis in children.

THE EXPERIENCE OF CIPRINOL USING IN NOSOCOMIAL PNEUMONIA TREATMENT AMONG THE SURGICAL DEPARTMENT PATIENTS

The efficacy of nosocomial pneumonia treatment by Ciprinol («KRKA», Slovenia) in surgical patients was studied. We find out data that Ciprinol has the high level of clinical efficacy and good profile of safety and microbiological activity for most common pathogens of nosocomial pneumonia.

EFFICACY OF OFLOXACIN IN TREATMENT OF PROGRESSIVELY WORSE FORMS OF MULTIDRUG RESISTANT LUNG TUBERCULOSIS

In open prospective randomised research included 36 patients with progressively worse disseminated destructive lung tuberculosis were studied bacteriostatic activity of blood against Mycobacterium tuberculosis (MBT) and clinical efficiency of ofloxacin. Is established, that the drug create in blood high bacteriostatic concentration during 3 hours after dosed 0,4 g, which is reduce up to moderate level after 6 hours. Using of ofloxacin allowed to remove growing progressively worse of tuberculosis at 100 % of the patients with multidrug resistant MBT and to achieve significant clinical and radiological improvement for first 2 months of treatment.

CLINICAL STUDY OF EFFICACY IN COMPLEX TREATMENT BY TAVANIC OF NONSPECIFIC INFLAMMATION IN PATIENTS WITH UROGENITAL TUBERCULOSIS

Adduced results of use levofloxacine (Tavanic, «Aventis Farma», France) in patients with urogenital tuberculosis for treatment of concomitant nonspecific inflammation. Noted statistically significant reduction or absentia of white blood cells and microorganisms in urine, secretion of prostate and ejaculate.

ANTIMICROBIAL ACTIVITY OF THE NEW DRUG VOSUREL

It was specified activity of the new drug Vosurel in addition to gram-positive and gram-negative museum specimens, just as clinical specimens with polyresistance to antibacterial drugs. Vosurel have keep own activity during long passage specimens in vitro. Represented data is prerequisite for examination of Vosurel as a potential antibacterial drug with wide range of antimicrobial activity.

EFFICACY OF PALLIATIVE RADIOTHERAPY IN COMBINATION WITH CLODRONATE FOR TREATMENT OF BONE METASTASES

The efficacy of combination of external beam irradiation and clodronate (Bonefos, «Schering AG», Germany) for palliation of painful bone metastases of breast carcinoma was studied. 
External beam irradiation was provided for 95 patients to a total dose 30 Gy/3 Gy/2 weeks by the «ROCUS»-units or linear accelerator with photons energy 6 MeV were used. Forty five women received clodronate intravenously during the first 5 sessions of radiotherapy to a total dose of 1500 mg/5 infusions, than they continued oral taking 1600 mg daily. Simultaneously usage of external irradiation and clodronate has a good tolerance, increases the analgesic effect of irradiation, and increases the remineralization of bone extracellular matrices. The usage of clodronate allowed stabling analgetic effect in 82,2±4,3% cases vs. to 46,0±3,9% in the group with external radiotherapy alone.

FEATURES OF PERSONALIZATED ADJUVANT POLYCHEMOTHERAPY UNDER BREAST CANCER

The analysis of results finding by cultivation explant of breast cancer in RPMI-1640-medium with adding of cytostatics (fluorouracil, cyclofosfamid, doxorubicin and epirubicin). By the results of sensitivity explanted tumor to cytostatics adjuvant chemotherapy was prescribed to patients. Materials of 2-years observation cited.

LABORATORY DIAGNOSTIC OF ROTAVIRUS INFECTION, PRINCIPLES AND METHODS

In the article were adduced laboratory diagnostics of rotavirus infection, modern principles, methods and algorithm of essay in different age-related groups were represented. Special attention was given on using modern methods of express-diagnostics of this infection with using highly sensitive and specific test-systems, which are requires availability of expensive equipment and materials, what's very important in conditions of applied virology laboratory. Focused one's attention on appraisal and understanding of investigation results, which were taking in consequence of using this methods.
