Some aspects of antibacterial sequential therapy of community-acquired pneumonia and the associated role of "respiratory" fluoroquinolone levofloxacin

The advantages of antibacterial sequential therapy of community-acquired pneumonia in comparison with conventional parenteral antibacterial therapy have been summarized. The novel "respiratory" fluoroquinolone levofloxacin have been characterized by high clinical and microbiological efficacy.

Modern treatment options in patients with metastatic liver tumors

We analyzed the literature data regarding current treatment options for secondary malignant hepatic neoplasms. Surgical resection remains the most effective method, however, only small percent of patients with liver metastases are estimated to be candidates for resection. Hepatic cryosurgery and transcatheter arterial chemoembolization are characterized by promising results regarding survival and may improve the outcome in suitable patients.

Application of minimal doses of the drugs in oncology

Literature review deals with the analysis of efficacy of minimal and sub-minimal doses of drugs and biologically active combinations, first of all anticancer compounds. The data regarding an administration of minimal doses of cytostatics and immunomodulators were studied. The conclusions about perspectives of that kind of studies for development of new compounds and methods of application of drugs were stated.

Drug allergy remains an urgent problem because of its high prevalence in population, especially in subjects often requiring long treatment courses. Three-staged procedure should be applied for timely diagnosis of drug allergy in medical institutions: 1st - profound study of medical history and administration of modified skin tests; 2nd - application of nasal allergen challenge; 3rd - complex laboratory examination.

The efficacy of moxifloxacin in infectious exacerbations of chronic obstructive bronchitis

Peculiarities of clinical course, endoscopic changes, dynamics of tensiometry and rheology parameters of serum and systemic changes of microcirculation were studied in 62 patients with chronic obstructive bronchitis. The conventional therapy was administered to 31 patient, the rest of the patients received the inductor of endogenous interferon - cycloferon. Positive outcomes of the disease registered in all patients, however more expressed clinical effect was noted cycloferon group. The application of the inductor of endogenous interferon promoted more favorable clinical course of disease, improvement of bronchial drainage function, more significant reduction of obstructive syndrome and decrease of severity of endobronchitis. The rate of improvement of parameters of the dynamic interphase tensiometry and rheology of serum, the system parameters of microcirculation in cycloferon group of patients exceeded the similar parameters in conventionally treated patients.

There was studied a susceptibility to ampicillin, chloramphenicol, tetracylin, nitrofurantoin, ciprofloxacin, gentamycin, imipenem and vancomycin of 30 strains of E. faecalis and 20 strains of E. fecium, isolated from the feaces of the newborns with perinatal diseases during January-June 2001, who had been hospitalized to intensive care departament of neonatalogical hospital.
There was revealed a high level and multiplicity of resistance of isolated from hospitalized patients enterococci. There were no significant differences in resistance pattern between E. faecalis and E. fecium. None of the isolates was vancomycin-resistant. Based on study resuts one could consider the hospital genesis of strains, colonizing the newborn bowel. The necessity of microbiological monitoring and approaches to administration of antibiotics for treatment of enterococcal infections in specific hospital were substanciated.

The effect of ATP-LONG on metabolic processes and structural and functional status of erythrocyte membranes in intraarterial polychemocherapy of the patients with locally advanced gastric cancer has been studied. It has been revealed that this medication had significant membrane stabilizing effect and promoted a decrease of toxicity of chemotherapy.

Clindamycin in treatment of purulent inflammatory diseases

The results of this clinical trial confirm high clinical efficacy of oral clindamycin in dosages 150-300 mg 4 times daily for 5-14 days in patients with paratonsillitis, paratonsillar abscess, chronic purulent maxillary sinusitis and exacerbation of purulent otitis media.

The methodical recommendations are devoted to the relevant problem of respiratory medicine - community-acquired pneumonia in adults. The up-to-date definition of pneumonia, epidemiological data regarding its prevalence, mortality and lethality have been presented. The etiology, pathogenesis and diagnostics of the disease have been characterized. There have been described the modern approaches in treatment of community-acquired pneumonia. A special emphasis has been put on the rational administration of the antibiotics. The authors have summarized the data regarding novel antibacterial medications and treatment regimens. The effectiveness of therapy evaluation criteria and examples of formulation of community-acquired pneumonia diagnosis have been stated.

The aspects of epidemiological situation on influenza in Ukraine for 2000/2001 and 2001/2002 seasons, as well as modern data regarding influenza virus morphology and antigen variability were presented. Available influenza vaccines were characterized. The risk groups for immunization in children, adolescents and adults were allocated. The peculiarities of immune response, adverse reactions, epidemiological and immunological effectiveness of attenuated vaccines were determined.
