The questions of epidemiology of lung diseases in Ukraine for the period of 1990-2000 were highlighted. The authors also described the main scientific achievements of the pulmonology for the 10 year period of Ukraine independence. The article highlights the modern approaches in antibacterial therapy of lower respiratory tract infections. The characteristics and the place of new "respiratory" fluoroquinolone levofloxacin (Tavanic) in mono- and combined therapy of community-aquired pneumonia and COPD exacerbation were presented.

The results of conducted study evidenced, that monotherapy with ampicillin/sulbactam (1,5 g 4 times daily IM during 8-12 days) was highly effective in treatment of patients aged <60 years with severe community-acquired pneumonia without concomitant diseases and severe complications.

The aim of the study was the analysis of amizon influence on erythrocytes acid resistance in vitro. It was established that amizon increased the erythrocytes acid resistance, inhibited by the products of pneumonia pathogens metabolism and lipid peroxidation.

A ROLE OF GRAMNEGATIVE FLORA AND S. AUREUS IN GENESIS OF PNEUMONIA IN SERVICEMEN OF REGULAR ARMY AND CHANGES OF SUSCEPTIBILITY OF THESE MICROORGANISMS TO ANTIBIOTICS

To define a role of gramnegative flora and S.aureus in genesis of pneumonia and to evaluate the susceptibility of these microorganisms to the most widespread antibiotics the data of a microbiological study of sputum of 1132 servicemen of regular army with pneumonia, treated in our hospital during 1987-1998, were analyzed. S. aureus and gramnegative flora were the most significant pathogens of pneumonia after pneumococcus in servicemen of regular army and caused a disease in 25,6% of cases (13,9% - gramnegative flora and 11,7% - S. aureus).
These microorganisms were resistant to penicillin and doxycycline. During the past years a decreased susceptibility of these microorganisms to ampicillin, streptomycin, kanamycin, gentamycin has been observed. The susceptibility of S. aureus to rifampicin, K. pneumoniae to levomycetin, and H. influenzae to erythromycin have increased.

THE SUSCEPTIBILITY TO ANTIBIOTICS OF the PATHOGENS, CAUSING
PURULENT-INFLAMMATORY DISEASES IN SURGICAL PRACTICE

The aim of the study was the analysis of susceptibility to antibiotics (including a novel Ukrainian antibiotic batumin) of microorganisms, causing purulent-inflammatory diseases in patients of surgical departments. It was established that the majority of bacteria were resistant to a number of antibiotics, which had been extensively used in clinical practice. Aerobes and facultative anaerobes were resistant to penicillin and susceptible to aminoglycosides; whereas obligate anaerobes were resistant to aminoglycosides and susceptible to penicillin.

STUDY OF FLURENIZIDE AND ISONIAZIDE MYCOBACTERIUM TUBERCULOSIS CROSS-RESISTANCE

For in vitro studies of cross-resistance we used a laboratory strain of Mycobacterium tuberculosis H37Rv (MBT) and isolated from patients both susceptible and resistante strains of MBT. The effect of flurenizide against these strains was studied. The criterion of MBT resistance to flurenizide in Levenshtain-Jensen media was a concentration of 1 mg/l. Flurenizide was active against all susceptible to isoniazide strains. Flurenizide was two times less active against susceptible to isoniazide strains then isoniazide. We revealed full cross-resistance between flurenizide and isoniazide - flurenizide was inactive against isoniazide-resistant strains.

CLINICAL EFFECTIVENESS AND SAFETY OF ETHAMBUTOL RECTAL SUPPOSITORIES IN TREATMENT OF PULMONARY TUBERCULOSIS PATIENTS

The results of clinical study of Ethambutol rectal suppositories (0,4 g, produced by "Monfarm", Ukraine) were evaluated. A limited clinical study was carried out in 65 patients with pulmonary tuberculosis. Ethambutol in tablets (0,4 g produced by Borschagivsky CPF, Ukraine) was chosen as a comparator. The obtained results allowed concluding, that both study medicines had similar effectiveness. In fact, Ethambutol in suppositories had better tolerability and adverse event profile.

THE PECULIARITIES OF HELICOBACTER PYLORI ERADICATION DURING TRIPLE THERAPY

The term of Helicobacter pylori eradication in 18 patients with chronic gastritis and duodenal peptic ulcer was studied. The patients received triple therapy using clarithromycin 1000 mg daily. The presence of Helicobacter pylori and the state of mucous membrane were evaluated endoscopically, histologically and cytologically. It appeared that the earliest stable positive effect was achieved on the third day of treatment in 44% of patients. On the 7th day it raised up to 83%. The endoscopic criterion of treatment effectiveness was an exacerbation of inflammatory process on the third day of the therapy. For young primarily treated patients with short case history it may be recommended an earlier discontinuation of the antibiotics under the gastroscopic and bacterioscopic control.

THE EXPERIENCE OF METHOTREXAT APPLICATION IN TREATMENT OF CHILDREN WITH ACUTE LYMPHOBLASTIC LEUKEMIA

Summary. The article presents the results of methotrexat application in 250 patients with acute lymphoblastic leukemia. This therapeutic program raises the effectiveness of polychemotherapy of these patients and works as a prophylaxis of specific CNS lesions. The article contains information on tolerability of methotrexat by the patients of different age groups and a description of the most frequent side effects of this medication.

SURVIVAL OF THE BREAST CANCER PATIENTS UNDER THE COMPLEX TREATMENT USING IMMUNOMODULATOR, DERIVED FROM LACTOBACILLUS DELBRUECKII

We analyzed a five year survival of the breast cancer patients at II - III stages, which underwent the complex treatment with domestically produced natural immunomodulator (NI) blasten. It was demonstrated that general and no-recurrence survival increased on 13% and 10% in comparison to the control group. The most prominent survival increase (24%) was noted in patients, treated with neoadjuvant therapy in combination with NI. A proposed dosing schedule of NI in the complex treatment of breast cancer did not stimulate the cancer progression. This study created a basis for administration of NI, derived from Lactobacillus Delbrueckii, in the complex treatment of breast cancer patients as a background medicine.

Summary. The current data on cytokines system performance and peculiarities of interferons (IF) mechanism of action are described in this article. The following effects of IF are presented: antiviral, antiproliferative, immunomodulative, and antibacterial. The classifications of therapeutic forms of IF and IF inductors are summarized. The characteristics of a low molecular weight IF inductor Cycloferone and its effects in patients with internal diseases are the main focus of this review.
